Introduction ‘What Matters Most?’ is a key part of the 5Ms framework in geriatric medicine with the goal of optimising patient centred care[1]. It aims to better align patient preferences and values with provision of healthcare and promote patient autonomy. Method The ‘What Matters Most?’ question was included as part of the Comprehensive Geriatric Assessment (CGA) undertaken by Geriatric Emergency Multidisciplinary Service (GEMS) in a model 3 Irish hospital. Patients over 75 years of age living with frailty were prioritised for review. Data was retrospectively reviewed over a 5 month period. Results: 606 CGAs were completed between January 1st and 31st May 2024. After excluding incomplete or duplicate records 474 were reviewed for thematic analysis. More than one theme was identified from each patient statement. Average age ?? and Clinical Frailty Score was ?? The most common theme was remaining at home on discharge and/or avoiding admission to hospital(33.8% (n=160)).Other themes included; family 26.3%(n=125), remaining independent 16.9%(n=80) and maintaining and optimising health. 16.2%(n=77). Only 2.7%(n=13) said that managing their presenting complaint was what mattered most. Key conclusions This review highlights the health outcomes that older people value and the ongoing need to provide adequate community supports to enable older patients to be discharged to and remain at home.. It also emphasises the importance of maintaining and promoting independence and optimising health and wellbeing in this population.
